Affiliated Eye Surgeons was established in Phoenix, Arizona in 1944 by Virgil Toland, M.D. At that time, the population was less than 70,000. Dr. Paul McFarland joined Dr. Toland in 1948. Dr. E. Fredrick Bloemker joined the practice in 1971. The other doctors have been brought into the practice since 1971. Affiliated Eye Surgeons is now the largest ophthalmology practice in Maricopa County. As the oldest established Ophthalmology practice in Phoenix, Affiliated Eye Surgeons offers its patients a sense of security that only experience and longevity can provide. The doctors of Affiliated Eye Surgeons are extremely proud of the fact that since its inception in 1944, there has never been a malpractice judgment against us. This is a great indicator of the quality of surgical and medical care given to our patients.

Affiliated Eye Surgeons offers a broad spectrum of eye care services to the metropolitan Phoenix area ranging from routine eye exams, cataract surgery, refractive surgery and laser surgery. We have performed more ocular surgical procedures since our inception than any other practice in the Phoenix area and with a staff of six ophthalmologists, three trained in subspecialties, our patients are ensured the highest quality eye care. Ocular examinations performed on patients at other practices are often not conducted by a board-certified Medical Doctor. Every patient that is seen at Affiliated Eye Surgeons has their entire ophthalmology examination done by a board-certified Medical Doctor.

Surgical Facilities:

We perform most of our surgery at St. Joseph's Hospital Outpatient Surgical Department. We also use the outpatient surgical facilities at Scottsdale Memorial Hospital and Good Samaritan Hospital. By using these excellent facilities we can provide an outpatient surgical facility convenient to you.

Hours:
8:00 a.m. - 5:00 p.m. Monday through Friday
Saturday hours available by appointment only.

Appointments:
Patients are seen by appointments only. If you find it necessary to cancel an appointment, please provide 24 hour's notice so we may schedule another patient. If you need an appointment, email appt@azeyes.com and provide us with your name and telephone number. A representative will call you shortly to schedule an appointment that is convenient for you. When you call our office, the phone will be answered by a real, live, caring person, not some machine. Using machines to answer the phones does save the practice money, but it can be terribly frustrating for the patients calling in.

Fees:
Charges for office visits and surgery are determined by the time spent and the complexity of the procedure. We've found our fees to be competitive with, and many times less than, average fees in the community. For surgical procedures including cataract surgery and laser refractive surgery, there are financing plans available. Affiliated Eye Surgeons currently use the Vision Fee Plan to assist patients with financing LASIK and other surgeries not covered by insurance.

Insurance Plans:
Our physicians are contracted with most of the popular PPO and HMO plans in the Valley. If we are contracted with your insurance company, please bring your card and we will be happy to file a claim on your behalf. Don't forget that many of your insurance plans require a referral from your primary care physician. It is your responsibility to obtain that referral. Medicare:
All of our doctors are Medicare providers. Medicare pays 80% of a set fee that we have agreed to accept for our services, with the remaining 20% paid by the patient or by co-insurance.

After-hours:
One of our doctors is on call at all times and can be reached in the case of an urgent problem by calling the regular office phone number, or by leaving a message with our answering service who will contact the doctor immediately. You can reach our answering service at 602-230-4860.

Emergencies:
If there is an emergency, such as a serious injury to your eye, go to the hospital emergency room for immediate care. Ask the hospital to contact us.

Prescription Refills:
Refills or other routine needs should be handled during regular office hours. Prompt refills are best handled by contacting your pharmacy who will notify us via fax of your prescription refill. Our fax number is 602-263-0778. In most cases we can turn requests around the same day.
